Which Maryland Domestic Relations Pro Se Forms Do You Need?

You should only file one complaint - do not file separate complaints for divorce, custody and child support. Only file a complaint for divorce if all of this applies. If you are not ready to file your divorce action, you can file a complaint for Custody to deal with both the custody and child support issues. If child support is the only issue, such as modification of child support or collection child support, then file the child support petition or contempt petition only.

Child Support

If you only want child support, and there is no current order for support you use these forms:


Dom. Rel. 1, Complaint for Child Support
Dom. Rel. 30, Financial Statement for Child Support  (Short Form) but
if you request either Alimony and/or a Dom. Rel. 32, Motion for Waiver of Pre-Payment of Fees then you must use Dom.Rel.31, Financial Statement
(Alimony or Child Support) (Long Form)
Dom. Rel. 55, Affidavit of Service (Private Process) or
Dom. Rel 56, Affidavit of Service (Certified Mail).
Dom. Rel. 51, Request for Master's Hearing (Filed after the Answer(Dom.Rel.50)
or if no Answer then:
File With Dom.Rel.54., Request for an Order for Default (except in Anne Arundel and Montgomery County Dom.Rel.51 is not used).
Form Dom.Rel.50, Answer to Complaint/Petition/Motion is filed by the other side in answer to the Complaint for Child Support.
